SPORTS PARTICIPATION PROCEDURE (effective May 2018)
The Northwestern Lehigh School District Sports Physical Examination procedure will ensure our student-athletes are receiving a comprehensive health physical examination prior to setting foot on the field or court. Additionally, our student-athletes and their parents/guardians will be better educated on understanding the risk of concussion, traumatic brain injury, and sudden cardiac arrest. Please be reminded that each student-athlete is responsible for securing their own physical at their own expense and that those physicals will be valid for the entire school year. Physicians cannot date the form prior to June 1 in order to be valid for the subsequent school year.
Below, please find links to the new sport physical forms packet adopted and sanctioned by the Pennsylvania Interscholastic Athletic Association (P.I.A.A.) in addition to the NWLSD Honor Code.
Instructions for completing the PIAA Physical Form Packet are as follows:
Sections #1, #2, #3, #4, & #5 must be completed by the parent/guardian and student-athlete
Section #2 requires signatures by each of the sports your son/daughter wishes to participate in during the school year and 5 signatures at the bottom of the page.
Sections #3, #4, & #5 requires 1 parent/guardian signature and 1 student-athlete signature at the bottom of the page.
Section #6 must be completed by a physician. PLEASE NOTE THAT THE PHYSICIAN’S SIGNATURE MUST BE ON SECTION #6 OF THIS FORM. NO OTHER FORM WILL BE ACCEPTED. PHYSICIANS SIGNATURE MUST BE DATED JUNE 1 OR LATER. The cost of the physical examination is the responsibility of the student and the student’s family. Any student who is approved for the “National School Lunch Program Meal Benefits” AND who does not have medical insurance, should contact the Athletic Office at (610) 298-8661 Ext. 2211 prior to attaining a physical examination as all costs associated will be paid for by the School District using our Team Physician.
Sections #7 only needs to be completed by the parent if student is participating in a subsequent sports season after the original sport physical paperwork has been completed. Section #8 only needs to be completed by the student’s physician if there was a significant injury is sustained after the original physical examination. Section #9 will be completed by school medical staff for wrestlers only.
The Athletic Honor Code must be read thoroughly and signed by both the student-athlete and the parent/guardian. The last page of the Code has room for signatures and only the page with the signatures needs to be submitted to the Athletic Office.
Once the Physical Form and the Athletic Honor Code have been completed by both the parent/guardian, student-athlete and the family physician, please send/deliver the completed forms to Northwestern Lehigh Athletic Department, 6493 Route 309, New Tripoli, PA 18066 or FAX it to (610) 298 – 3102 or scan/email it to firstname.lastname@example.org. All documents must be submitted prior to the first day of practice or the student will NOT be eligible to tryout/practice.
